Effective today, all residents or their substitute decision maker are able to designate up to 3 Essential Family Caregivers (previously 2).
Only one EFC is able to visit at a time.
EFC agreements for the Manor and the Gardens will now be processed by reception. You can return your EFC agreement in the following ways;
- Drop off at reception
- Scanned and emailed to reception@shalommanor.mystagingwebsite.com
- Faxed to 905-945-1211
The EFC agreement, important information regarding EFC visits and required education to review before your first visit can be obtained from reception. Please note: EFC agreements will only be processed Monday-Friday 8-4. Please provide us with 1 business days’ notice in advance of your first visit to the home so we can process the paperwork and make your nametag.
Parking-IMPORTANT: More EFC’s coming into the building will limit the available parking at Shalom, therefore, we ask that on weekdays you do not arrive for your EFC visit during PSW shift change times, which is 1:45-2:15. If mobility is not an issue for you, we also encourage you to park along Central Ave., as there is ample parking with sidewalk access to Shalom.
