The Admission Process: In Ontario, all admissions to long-term care are coordinated through Ontario Health atHome (formerly known as CCAC or the LHIN). If you feel you may be ready for long-term care, your first step is to contact Ontario Health atHome at 1-800-810-0000. Once you connect with Ontario Health atHome, a Care Coordinator will be assigned to you. They will complete an in-person assessment to better understand your care needs and determine whether long-term care is appropriate. If you are eligible, they will support you in completing your application and guide you through selecting your preferred home(s), which may include Shalom Manor.

Each long-term care home you select will review your application to confirm whether they are able to meet your care needs. If you are accepted but a bed is not immediately available, your name will be placed on a centralized waiting list managed by Ontario Health atHome. Shalom Manor is not able to provide information about your position on the waiting list or estimate timelines for admission offers. You will continue to work directly with your Care Coordinator at Ontario Health atHome until a placement is accepted.

Cultural Admissions: Our Home is rooted in and supported by the Dutch Reformed community, and we strive to serve individuals who share or value this tradition. As part of Ontario’s ‘modified placement model’, Shalom Manor is able to prioritize admissions for individuals who align with our Reformed faith and community values and who have crisis status on the waiting list. If you are part of our supporting community, we encourage you to speak with your Placement Coordinator about this during the application process.
